And although insulin saves lives, <\/span><b>it&#8217;s still a stopgap<\/b><span style=\"font-weight: 300;\">: it doesn&#8217;t cure the disease or prevent the associated long-term complications.<\/span><\/p>\n<p><span style=\"font-weight: 300;\">On World Diabetes Day, we explore new lines of research that seek to go one step further: to release patients from their dependence on chronic treatments, <\/span><b>improve their quality of life and perhaps one day find a definitive cure<\/b><span style=\"font-weight: 300;\">.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3><b><i>A 21st-century pandemic<\/i><\/b><\/h3>\n<p><span style=\"font-weight: 300;\">In 2024, 589 million adults were affected by diabetes and, according to the <\/span><a href=\"https:\/\/diabetesatlas.org\/data-by-location\/global\/\"><span style=\"font-weight: 300;\">International Diabetes Federation (IDF)<\/span><\/a><span style=\"font-weight: 300;\">, the future projections are stark: 643 million by 2030 and 853 million by 2050.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 300;\">Of the affected population, more than 90% have type 2 (T2D) and between 5-8% have type 1 (T1D). It&#8217;s important to distinguish between these two main types.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 300;\">Type 1 is an <\/span><b>autoimmune<\/b><span style=\"font-weight: 300;\"> disease caused by a combination of genetic and environmental factors that trigger an abnormal immune response: the immune system attacks the pancreas&#8217;s \u03b2 cells, which are responsible for producing insulin. &#8220;As a result, local inflammation occurs and these cells gradually die, ultimately <\/span><b>stopping the patient from producing enough <\/b><span style=\"font-weight: 400;\">insulin<\/span><span style=\"font-weight: 300;\">&#8221; explains <\/span><a href=\"https:\/\/caixaresearch.org\/en\/caixaresearch-health-call-projects-2024-diabetes-therapy\"><span style=\"font-weight: 400;\">Lorenzo Pasquali,<\/span><\/a><span style=\"font-weight: 300;\"> a researcher from the &#8220;la Caixa&#8221; Foundation network in the Department of Medicine and Life Sciences at Pompeu Fabra University (UPF) in Barcelona.\u00a0<\/span><\/p>\n<div id=\"attachment_11742\" style=\"width: 410px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-11742\" class=\"wp-image-11742\" src=\"https:\/\/blog.caixaresearch.org\/wp-content\/uploads\/2025\/11\/Blog_DM-Diabetes4-300x169.jpg\" alt=\"\" width=\"400\" height=\"225\" srcset=\"https:\/\/blog.caixaresearch.org\/wp-content\/uploads\/2025\/11\/Blog_DM-Diabetes4-300x169.jpg 300w, https:\/\/blog.caixaresearch.org\/wp-content\/uploads\/2025\/11\/Blog_DM-Diabetes4-1024x576.jpg 1024w, https:\/\/blog.caixaresearch.org\/wp-content\/uploads\/2025\/11\/Blog_DM-Diabetes4-768x432.jpg 768w, https:\/\/blog.caixaresearch.org\/wp-content\/uploads\/2025\/11\/Blog_DM-Diabetes4-1536x864.jpg 1536w, https:\/\/blog.caixaresearch.org\/wp-content\/uploads\/2025\/11\/Blog_DM-Diabetes4-2048x1152.jpg 2048w\" sizes=\"auto, (max-width: 400px) 100vw, 400px\" \/><p id=\"caption-attachment-11742\" class=\"wp-caption-text\"><em>Lorenzo Pasquali<\/em><\/p><\/div>\n<p><span style=\"font-weight: 300;\">On the other hand, in type 2 diabetes the body may not produce enough insulin to meet its needs or, even if it does, <\/span><b>the body may not be able to use it properly <\/b><span style=\"font-weight: 300;\">due to a combination of risk factors, including obesity, a sedentary lifestyle and genetic predisposition.<\/span><\/p>\n<p><span style=\"font-weight: 300;\">In both cases the result is excess blood sugar, which in the long term can cause ulcers, eyesight problems, kidney damage, strokes and a weakened immune system, seriously affecting the patient&#8217;s health.\u00a0\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3><b><i>The invisible burden of treatment<\/i><\/b><\/h3>\n<p><span style=\"font-weight: 300;\">&#8220;For type 1 patients, standard treatment involves <\/span><b>multiple daily injections or insulin pumps, as well as the continuous monitoring of glucose levels<\/b><span style=\"font-weight: 300;\">&#8221; explains Pasquali. &#8220;It&#8217;s a demanding routine that requires constant attention and this has an impact on the mental health and daily lives of those affected&#8221;.<\/span><\/p>\n<p><span style=\"font-weight: 300;\">In type 2 diabetes, the range of potential treatments is more diverse and depends on the clinical characteristics of each patient. &#8220;It usually starts with <\/span><b>lifestyle <\/b><span style=\"font-weight: 400;\">changes<\/span><span style=\"font-weight: 300;\"> such as diet, exercise and weight control, followed by drugs that lower the blood glucose, such as metformin or, more recently, <\/span><a href=\"https:\/\/eurjmedres.biomedcentral.com\/articles\/10.1186\/s40001-024-02241-4\"><span style=\"font-weight: 300;\">SGLT-2 inhibitors and GLP-1 receptor agonists<\/span><\/a><span style=\"font-weight: 300;\">&#8221; notes the researcher. &#8220;But when these options fail, insulin once again becomes the last resort&#8221;.<\/span><\/p>\n<p><span style=\"font-weight: 300;\">Furthermore, despite current advances, insulin <\/span><b>can&#8217;t replicate the precision with which the pancreas regulates glucose levels<\/b><span style=\"font-weight: 300;\">. &#8220;This can lead to hypoglycaemia or complications which, over time, will affect the patient&#8217;s health&#8221; explains Pasquali.\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3><b><i>A lifelong solution<\/i><\/b><\/h3>\n<p><span style=\"font-weight: 300;\">&#8220;The large pharmaceutical companies develop many more solutions for type 2 diabetes because the market is much bigger&#8221; said Esther Latres, Director of the Breakthrough T1D Foundation, <\/span><a href=\"https:\/\/mediahub.fundacionlacaixa.org\/en\/research-health\/health\/medical-research\/2024-10-03\/esther-latres-diabetes-type-1-6474.html\"><span style=\"font-weight: 300;\">in this interview<\/span><\/a><span style=\"font-weight: 300;\">. These are known as &#8216;cell replacement therapies&#8217; and they&#8217;re based on the transplantation of pancreatic islets or \u03b2 cells derived from stem cells. The results are preliminary but already point to <\/span><b>a possible functional cure for the disease.<\/b><\/p>\n<p><span style=\"font-weight: 300;\">&#8220;The challenge is ensuring that the new cells survive the immune system&#8217;s attack without the need for immunosuppressants&#8221; explains Pasquali. His team is studying how inflammation and genetic predisposition influence the destruction of \u03b2 cells in order to design <\/span><b>more resistant versions<\/b><span style=\"font-weight: 300;\">. &#8220;We&#8217;ve already identified genes that could reinforce this protection and are testing genetic modifications with promising results&#8221; he adds.<\/span><\/p>\n<p><span style=\"font-weight: 300;\">The latest advances confirm this potential. In August 2025, two studies published in the <\/span><i><span style=\"font-weight: 300;\">New England Journal of Medicine <\/span><\/i><span style=\"font-weight: 300;\">showed that \u03b2 cells obtained from stem cells can produce insulin for months without the need for immunosuppressive drugs. Although the trials are still small, they represent a milestone. &#8220;These are proof-of-concept studies that demonstrate that <\/span><b>a functional cure is possible<\/b><span style=\"font-weight: 300;\">&#8221; says Pasquali. &#8220;The next challenge will be to ensure their safety, durability and accessibility so they can reach all patients&#8221;.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3><b><i>Towards a new era<\/i><\/b><\/h3>\n<p><span style=\"font-weight: 300;\">Diabetes research augurs a future of new therapies capable of restoring the body&#8217;s natural insulin production and reducing dependence on external treatments.<\/span><\/p>\n<p><span style=\"font-weight: 300;\">&#8220;We may still be a long way off from a universal, safe and accessible cure&#8221; Pasquali acknowledges. &#8220;But we can already envisage a future in which patients with type 1 diabetes can live for long periods of time without insulin and with a better quality of life&#8221;.<\/span><\/p>\n<p><span style=\"font-weight: 300;\">A century after its discovery, insulin continues to save millions of lives.
