A<\/span> questa trobada, celebrada el passat mes de setembre al CNIO, va reunir matem\u00e0tics, f\u00edsics i bioinform\u00e0tics de tot el m\u00f3n per debatre sobre els tractaments personalitzats contra el c\u00e0ncer gr\u00e0cies al Big Data.\u00a0<\/span><\/div>\n<div> \u00a0<\/div>\n<div>\n<p dir=\"ltr\"> <strong>Quin paper t\u00e9 la bioinform\u00e0tica en l\u2019estudi del c\u00e0ncer?<\/strong><\/p>\n<p dir=\"ltr\"> <span id=\"docs-internal-guid-bb286d16-7fff-0729-43c3-b82ff3b96b75\">Sabem que el c\u00e0ncer no \u00e9s una \u00fanica malaltia, sin\u00f3 diverses. Un c\u00e0ncer de pulm\u00f3 i un c\u00e0ncer de mama no s\u00f3n iguals, ni en el seu desenvolupament ni a escala molecular. Tampoc no s\u00f3n iguals dos tumors del mateix c\u00e0ncer en pacients diferents, ni tan sols les c\u00e8l\u00b7lules d\u2019un mateix tumor.<\/span><\/p>\n<p dir=\"ltr\"> <span id=\"docs-internal-guid-bb286d16-7fff-0729-43c3-b82ff3b96b75\">Tota aquesta heterogene\u00eftat comporta la necessitat de fer servir ter\u00e0pies diferents i espec\u00edfiques en cada cas. Actualment disposem de molt\u00edssimes dades moleculars que, mitjan\u00e7ant la bioinform\u00e0tica, volem fer servir per trobar la combinaci\u00f3 de f\u00e0rmacs m\u00e9s efica\u00e7 per a cada pacient.<\/span><\/p>\n<p dir=\"ltr\"> <strong>De quines eines disposem per estudiar aquesta heterogene\u00eftat?<\/strong><\/p>\n<p dir=\"ltr\"> <span id=\"docs-internal-guid-bb286d16-7fff-0729-43c3-b82ff3b96b75\">Des de fa uns anys, est\u00e0 emergint una manera molt potent de caracteritzar les poblacions cel\u00b7lulars, tant en l\u2019\u00e0mbit de l\u2019expressi\u00f3 g\u00e8nica com de les mutacions al DNA. Amb la bi\u00f2psia d\u2019un tumor, podem mesurar l\u2019abund\u00e0ncia de les diverses poblacions cel\u00b7lulars. I aix\u00ed, gr\u00e0cies a diverses t\u00e8cniques computacionals, predir com variaran en el futur.<\/span><\/p>\n<p dir=\"ltr\"> <strong>Aix\u00f2 vol dir que podrem predir l\u2019evoluci\u00f3 dels tumors amb models matem\u00e0tics?<\/strong><\/p>\n<p dir=\"ltr\"> <span id=\"docs-internal-guid-bb286d16-7fff-0729-43c3-b82ff3b96b75\">S\u00ed i no. Actualment hi ha metodologies que permeten estimar la superviv\u00e8ncia d\u2019un pacient o si t\u00e9 predisposici\u00f3 a desenvolupar un tumor concret. No obstant aix\u00f2, la nostra capacitat predictiva encara \u00e9s limitada i, per exemple, con\u00e8ixer l\u2019evoluci\u00f3 tumoral encara est\u00e0 en fases inicials.\u00a0<\/span><\/p>\n<p dir=\"ltr\"> <span id=\"docs-internal-guid-bb286d16-7fff-0729-43c3-b82ff3b96b75\">L\u2019evoluci\u00f3 d\u2019un tumor \u00e9s com la de les esp\u00e8cies: sota una pressi\u00f3 selectiva, determinades alteracions desapareixen i altres, les que poden proporcionar un avantatge al tumor, es tendeixen a preservar. \u00c9s el que s\u2019anomena <\/span>clonalitat tumoral. El problema del tumor \u00e9s que la seva evoluci\u00f3, i per tant adaptaci\u00f3, \u00e9s molt r\u00e0pida, i predir les alteracions que es produiran no \u00e9s f\u00e0cil. De fet, \u00e9s un tema de molt inter\u00e8s entre la comunitat investigadora.\u00a0<\/p>\n<p dir=\"ltr\"> <strong>Quines implicacions pot tenir entendre millor la clonalitat tumoral en els tractaments contra el c\u00e0ncer?<\/strong><\/p>\n<p dir=\"ltr\"> <span id=\"docs-internal-guid-bb286d16-7fff-0729-43c3-b82ff3b96b75\">Doncs molt\u00edssimes. Hem ent\u00e8s que aquesta clonalitat, l\u2019heterogene\u00eftat del tumor, \u00e9s la que acaba entre d\u2019altres coses generant una resist\u00e8ncia als tractaments. Quan a un pacient se li administra un tractament dirigit a una determinada alteraci\u00f3, s\u2019eliminen les c\u00e8l\u00b7lules que contenen aquesta mutaci\u00f3 espec\u00edfica. No obstant aix\u00f2, les que no la tenen, emergeixen i n\u2019apareixen nous clons.<\/span><\/p>\n<p dir=\"ltr\"> <strong>Aquest \u00e9s el problema que voleu tractar amb la vostra col\u00b7laboraci\u00f3 amb el MIT?<\/strong><\/p>\n<p dir=\"ltr\"> <span id=\"docs-internal-guid-bb286d16-7fff-0729-43c3-b82ff3b96b75\">Efectivament. Treballem amb el grup de Manolis Kellis, del MIT, per comprendre les bases biol\u00f2giques de la resist\u00e8ncia a tractaments amb immunoter\u00e0pia. L\u2019objectiu final \u00e9s proposar alternatives terap\u00e8utiques per als pacients que la pateixen.<\/span><\/p>\n<p dir=\"ltr\"> <span id=\"docs-internal-guid-bb286d16-7fff-0729-43c3-b82ff3b96b75\">L\u2019equip de Kellis estudia el genoma hum\u00e0, mentre que nosaltres, al CNIO, desenvolupem metodologies com\u00a0<\/span><a href=\"https:\/\/www.pandrugs.org\/\" rel=\"noopener noreferrer\" target=\"_blank\">Pandrugs<\/a>, un algoritme per predir tractaments personalitzats a partir de les alteracions gen\u00e8tiques detectades en cada pacient.<\/p>\n<div> <img decoding=\"async\" alt=\"\" src=\"https:\/\/blog.caixaresearch.org\/wp-content\/uploads\/2020\/04\/ca\/bioinformatica_cnio.jpg\" style=\"width: 400px; height: 251px;\"\/><\/div>\n<div> <span style=\"font-size: 11px;\">Unitat de Bioinform\u00e0tica del Centre Nacional d\u2019Investigacions Oncol\u00f2giques (CNIO). A les pacients amb c\u00e0ncer de mama amb una alteraci\u00f3 determinada, per exemple, se\u2019ls dona un tractament espec\u00edfic.<\/span><\/span><\/p>\n<p dir=\"ltr\"> <span style=\"font-size:12px;\"><span id=\"docs-internal-guid-86ab84ff-7fff-6d1b-3a21-c1bca7593733\">Ja tenim informaci\u00f3 molecular per guiar un tractament, per\u00f2 encara no hem arribat a exprimir-ne el potencial m\u00e0xim. Per a aix\u00f2, necessitem dades de qualitat, estandarditzades i actualitzades, sobre el perfil gen\u00f2mic d\u2019un pacient, dades de salut i hist\u00f2ries cl\u00edniques.\u00a0<\/span><\/span><\/p>\n<p dir=\"ltr\"> <span style=\"font-size:12px;\"><strong>Quin paper ha tingut el finan\u00e7ament obtingut a trav\u00e9s de \u201dla Caixa\u201d en el desenvolupament de la recerca?<\/strong><\/span><\/p>\n<p dir=\"ltr\"> <span style=\"font-size:12px;\"><span id=\"docs-internal-guid-86ab84ff-7fff-6d1b-3a21-c1bca7593733\">\u00c9s gr\u00e0cies a l\u2019ajuda de \u201dla Caixa\u201d que podem col\u00b7laborar amb un grup del MIT expert en l\u2019estudi del genoma. Ells ens proporcionen dades de qualitat.
