De ser una malaltia mortal va passar a ser un <\/span><b>trastorn cr\u00f2nic, complex, per\u00f2 manejable<\/b><span style=\"font-weight: 400;\">.<\/span><span style=\"font-weight: 300;\"> I avui, cent anys despr\u00e9s d\u2019aquell descobriment, la insulina continua sent el tractament de refer\u00e8ncia.<\/span><\/p>\n<p><span style=\"font-weight: 300;\">Tot i els grans aven\u00e7os tecnol\u00f2gics \u2014bombes d\u2019insulina intel\u00b7ligents, sensors de glucosa, algoritmes de control\u2014, el principi terap\u00e8utic no ha canviat: substituir una hormona que l\u2019organisme ja no pot produir o utilitzar correctament. I \u00e9s que, encara que salva vides, <\/span><b>la insulina continua sent un pegat<\/b><span style=\"font-weight: 300;\">: no cura la malaltia ni evita les complicacions que comporta a llarg termini.<\/span><\/p>\n<p><span style=\"font-weight: 300;\">Amb motiu del Dia Mundial de la Diabetis explorem noves l\u00ednies de recerca dirigides a fer un pas m\u00e9s: alliberar els pacients de la depend\u00e8ncia del tractament cr\u00f2nic, i tamb\u00e9 <\/span><b>millorar la seva qualitat de vida i, potser algun dia, aconseguir una cura definitiva<\/b><span style=\"font-weight: 300;\">.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3><b><i>Una pand\u00e8mia del segle XXI<\/i><\/b><\/h3>\n<p><span style=\"font-weight: 300;\">El 2024, la diabetis ja afectava 589 milions d\u2019adults. I les projeccions de futur s\u00f3n contundents: 643 milions el 2030 i 853 milions el 2050 segons la <\/span><a href=\"https:\/\/diabetesatlas.org\/es\/data-by-location\/global\/\"><span style=\"font-weight: 300;\">Federaci\u00f3 Internacional de Diabetis (<\/span><span style=\"font-weight: 300;\">FID)<\/span><\/a><span style=\"font-weight: 300;\">.<\/span><\/p>\n<p><span style=\"font-weight: 300;\">De la poblaci\u00f3 afectada, m\u00e9s del 90\u2009% presenta el tipus 2 (DT2), i entre el 5 i el 8\u2009%, el tipus 1 (DT1). \u00c9s important distingir entre aquests dos tipus principals.<\/span><\/p>\n<p><span style=\"font-weight: 300;\">En el cas del tipus 1, es tracta d\u2019una malaltia <\/span><b>autoimmune<\/b><span style=\"font-weight: 300;\">, causada per la combinaci\u00f3 de factors gen\u00e8tics i ambientals, i que desencadena una resposta immunit\u00e0ria an\u00f2mala: el mateix sistema immunitari ataca les c\u00e8l\u00b7lules \u03b2 del p\u00e0ncrees, responsables de produir insulina. \u00abCom a resultat es produeix inflamaci\u00f3 local i aquestes c\u00e8l\u00b7lules moren de manera progressiva, cosa que finalment <\/span><b>impedeix que el pacient generi prou insulina<\/b><span style=\"font-weight: 300;\">\u00bb, explica <\/span><a href=\"https:\/\/caixaresearch.org\/ca\/convocatoria-caixaresearch-investigacio-2024-projecte-terapia-diabetis\"><span style=\"font-weight: 400;\">Lorenzo Pasquali<\/span><\/a><span style=\"font-weight: 300;\">, investigador de la xarxa Fundaci\u00f3 \u201dla Caixa\u201d al Departament de Medicina i Ci\u00e8ncies de la Vida de la Universitat Pompeu Fabra (UPF) de necessitats o b\u00e9, tot i que en produeix, <\/span><b>no \u00e9s capa\u00e7 d\u2019utilitzar-la de manera adequada<\/b><span style=\"font-weight: 300;\"> a causa d\u2019una combinaci\u00f3 de factors de risc, entre els quals hi ha l\u2019obesitat, el sedentarisme i la predisposici\u00f3 gen\u00e8tica.<\/span><\/p>\n<p><span style=\"font-weight: 300;\">En tots dos casos es produeix un exc\u00e9s de sucre en sang que a llarg termini pot causar ulceracions, problemes visuals, danys renals, accidents vasculars i debilitament del sistema immunitari, que afecta greument la salut del pacient.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3><em><b>La c\u00e0rrega invisible del tractament<\/b><\/em><\/h3>\n<p><span style=\"font-weight: 300;\">\u00abPer als pacients amb tipus 1, el tractament est\u00e0ndard implica <\/span><b>injeccions m\u00faltiples a diari o bombes d\u2019insulina, a m\u00e9s del monitoratge continu de la glucosa<\/b><span style=\"font-weight: 300;\">\u00bb, explica Pasquali. \u00ab\u00c9s una rutina exigent que requereix una atenci\u00f3 constant i que impacta en la salut mental i en la vida quotidiana de les persones afectades\u00bb.<\/span><\/p>\n<p><span style=\"font-weight: 300;\">En el tipus 2, el panorama terap\u00e8utic \u00e9s m\u00e9s divers i dep\u00e8n de les caracter\u00edstiques cl\u00edniques de cada pacient. \u00abAcostuma a comen\u00e7ar amb <\/span><b>canvis en l\u2019estil de vida<\/b><span style=\"font-weight: 300;\"> \u2014dieta, exercici, control del pes\u2014 seguits per f\u00e0rmacs que disminueixen la glucosa en sang, com la metformina o, m\u00e9s recentment els <\/span><a href=\"https:\/\/eurjmedres.biomedcentral.com\/articles\/10.1186\/s40001-024-02241-4\"><span style=\"font-weight: 300;\">inhibidors de SGLT2 i agonistes de GLP-1<\/span><\/a><span style=\"font-weight: 300;\">\u00bb, apunta l\u2019investigador. \u00abPer\u00f2 quan aquestes opcions fallen, la insulina torna a ser el recurs final\u00bb.<\/span><\/p>\n<p><span style=\"font-weight: 300;\">A m\u00e9s, malgrat i els aven\u00e7os actuals, la insulina <\/span><b>no aconsegueix replicar la precisi\u00f3 amb la qual el p\u00e0ncrees regula els nivells de glucosa<\/b><span style=\"font-weight: 300;\">. \u00abAix\u00f2 pot provocar hipogluc\u00e8mies o complicacions que, amb el pas del temps, afectaran la salut del pacient\u00bb, afirma Pasquali.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3><b><i>Una soluci\u00f3 per a tota la vida<\/i><\/b><\/h3>\n<p><span style=\"font-weight: 300;\">\u00abLes grans farmac\u00e8utiques desenvolupen moltes m\u00e9s ter\u00e0pies i dispositius per a la diabetis de tipus 2 perqu\u00e8 el mercat \u00e9s molt m\u00e9s gran\u00bb, apunta en <\/span><a href=\"https:\/\/mediahub.fundacionlacaixa.org\/ca\/investigacio-salut\/salut\/investigacio-medica\/2024-10-03\/esther-latres-diabetis-tipus-1-6474.html\"><span style=\"font-weight: 300;\">aquesta entrevista<\/span><\/a><span style=\"font-weight: 300;\"> Esther Latres, directora de la fundaci\u00f3 Breakthrough T1D. Aquestes estrat\u00e8gies, conegudes com a <\/span><i><span style=\"font-weight: 300;\">ter\u00e0pies de reempla\u00e7ament cel\u00b7lular<\/span><\/i><span style=\"font-weight: 300;\">, es basen en el trasplantament d\u2019illots pancre\u00e0tics o de c\u00e8l\u00b7lules \u03b2 derivades de c\u00e8l\u00b7lules mare. Els resultats s\u00f3n incipients, per\u00f2 ja apunten a <\/span><b>una possible cura funcional de la malaltia<\/b><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 300;\">\u00abEl repte \u00e9s aconseguir que les noves c\u00e8l\u00b7lules sobrevisquin a l\u2019atac del sistema immunitari sense necessitat d\u2019immunosupressors\u00bb, explica Pasquali. El seu equip estudia com influeixen la inflamaci\u00f3 i la predisposici\u00f3 gen\u00e8tica en la destrucci\u00f3 de les c\u00e8l\u00b7lules \u03b2 per dissenyar <\/span><b>versions m\u00e9s resistents<\/b><span style=\"font-weight: 300;\">. \u00abJa hem identificat gens que podrien refor\u00e7ar aquesta protecci\u00f3 i estem provant modificacions gen\u00e8tiques amb resultats prometedors\u00bb, afegeix.<\/span><\/p>\n<p><span style=\"font-weight: 300;\">Ela aven\u00e7os m\u00e9s recents confirmen aquest potencial. L\u2019agost del 2025, dos estudis publicats al <\/span><i><span style=\"font-weight: 300;\">New England Journal of Medicine<\/span><\/i><span style=\"font-weight: 300;\"> van demostrar que les c\u00e8l\u00b7lules \u03b2 obtingudes a partir de c\u00e8l\u00b7lules mare poden produir insulina durant mesos sense necessitat de f\u00e0rmacs immunosupressors. Tot i que els assajos encara s\u00f3n redu\u00efts, representen una fita. \u00abS\u00f3n proves de concepte que demostren que <\/span><b>una curaci\u00f3 funcional \u00e9s possible<\/b><span style=\"font-weight: 300;\">\u00bb, afirma Pasquali. \u00abEl proper repte ser\u00e0 garantir-ne la seguretat, la durabilitat i l\u2019accessibilitat perqu\u00e8 puguin arribar a tots els pacients\u00bb.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3><b><i>Cap a una nova era<\/i><\/b><\/h3>\n<p><span style=\"font-weight: 300;\">L\u2019horitz\u00f3 de la investigaci\u00f3 en diabetis apunta cap a noves ter\u00e0pies capaces de restaurar la producci\u00f3 natural d\u2019insulina i reduir la depend\u00e8ncia de tractaments externs.<\/span><\/p>\n<p><span style=\"font-weight: 300;\">\u00abPotser encara estem lluny d\u2019una cura universal, segura i accessible\u00bb, reconeix Pasquali. \u00abPer\u00f2 ja albirem un futur en el qual els pacients amb diabetis de tipus 1 podran viure llargs per\u00edodes sense insulina i amb millor qualitat de vida\u00bb.<\/span><\/p>\n<p><span style=\"font-weight: 300;\">Un segle despr\u00e9s del seu descobriment, la insulina continua salvant milions de vides.
